PPTCT

In 2001, during the UN General Assembly Special Session ( UNGASS) on HIV/AIDS, the member states committed themselves to reducing the proportion of children infected through mother to child transmission (MTCT)  by 20% and 50% by 2005 and 2010 respectively.  Since then , globally, a large number of countries including India have launched Prevention of  Mother to Child Transmission (PMTCT) programs . However, by the end of 2003, only 8% of women globally had access to PMTCT services and two thirds of all countries implementing PMTCT programs had not passed the 20% coverage mark.

 

In India, feasibility of implementing Prevention of Parent to Child Transmission Services (PPTCT) services was studied in 5 states. Then, PPTCT interventions were launched in six high prevalence states including Maharashtra in November 2002 through the existing MCH services. Scaling up of these services has been planned in a gradual manner starting from medical colleges in high prevalence states and going to district hospitals in these states ; followed by medical colleges in low prevalence ( vulnerable) states and district hospitals in these states. The ultimate goal is to provide PPTCT services to the populations at the village level.

 

As of May 2006, Mumbai has scaled up the PPTCT services in 45 centers:

1 Government Medical college,

3 Municipal Medical colleges ,

1 Private Medical college,

13 Peripheral Hospitals,

20 Municipal Maternity Homes.

2 Urban Health Centres

5 ESIS Hospitals

As of date, the services are provided only through the health facilities. The HIV sero-prevalence in pregnant women antenatal clinics is estimated to be 1.1 % in 2006,

 

The PPTCT Services include:

Pre – Test counseling of registered ANCs.

Testing for HIV, with consent.

Post - Test counseling & handing over the report.

Follow up & care of HIV Positive mothers.

Administration of Nevirapine* to HIV positive mother during first stage of labour.

Administration of Nevirapine* to Baby of HIV positive mother, after birth within 2 days.

Follow up & continuing care & support to mother & baby.

Testing of the Baby of HIV Positive Mother at 18 months of age.

·    Spouse counseling & testing is also encouraged.

* Anti-retroviral drug used in PPTCT programme.

Activities of MDACS in the PPTCT programme :

Trainings of various cadres of Medical & Paramedical Personnel have been conducted.

I.E.C. material for PPTCT has been developed.

Regular supply of Testing Kits to these centers is ensured.

Regular supply of Nevirapine to these centers is ensured.

Monitoring & Evaluation of these centers regularly.

Scaling up the services & maintaining quality of services.

Establishing linkages with various services to strengthen the care & support.
